Coba Toxicology is committed to protect our patients' information. All information collected by our company is protected and confidential. Beyond your Personal Health Information, we will not sell, disclose or transmit any other personal information about you to any third party that is not authorized to receive that information. Unless you decide to opt out, we will use de-identified data for statistical research. Your privacy will be protected to the full extent of the law.

Patient data will be maintained on HIPAA-compliant servers or files to ensure the privacy of your information. Transmitted data will be encrypted. Coba Toxicology will responsibly provide physical security measures to protect written and logistical documents. Documents that are not required to be retained such as medical and transaction documents will be destroyed securely.

It is recommended that you, the patient, also take necessary steps to protect your privacy. It is not advisable to disclose Personal Health Information or other private information in non-secure email. Physical documents should be kept secured at all times. Destroy personal documents that do not need to be retained.

At your request, Coba Toxicology is required by law to provide copies of your genetic testing records. We recommend that you review those records with a qualified medical professional to be adequately interpreted. Unless advised to do so by your doctor, do not discontinue or alter your prescribed medications or dosages based on data provided by Coba Toxicology.
